What is an ACL Injury?

An ACL (anterior cruciate ligament) injury is a common knee injury, often caused by sudden stops, changes in direction, or trauma. The ACL is a crucial ligament that stabilizes your knee. When it's torn or damaged, it can cause pain, swelling, and instability.

ACL Surgery Options and Treatment

Treatment for an ACL injury depends on the severity of the tear and your activity level. At Paras Hospital, we always consider conservative treatments first for partial tears or less active patients, and surgical reconstruction for complete tears or those seeking a return to high-level activity​.

  • Non-Surgical Management: If the ACL is sprained or partially torn and the knee is still stable, or if the patient is older or less active, surgery may be avoidable. Conservative options include rest, ice, compression, and elevation (RICE) to reduce swelling, use of a knee brace for support, and physical therapy. Targeted physiotherapy can strengthen the surrounding muscles (especially the quadriceps and hamstrings) to compensate for the injured ACL and improve stability​​. Many patients with partial ACL tears respond well to these measures, regaining function without surgery. Our rehabilitation specialists will design a customized exercise program to help restore knee strength and range of motion while protecting the healing ligament. Regular follow-ups are scheduled to monitor knee stability.
  • ACL Reconstruction Surgery: For complete ACL tears or cases of persistent knee instability, surgery is often recommended to restore full function​. The gold-standard procedure is arthroscopic ACL reconstruction, a minimally invasive surgery where the torn ligament is replaced with a tissue graft​. During the surgery, the orthopedic surgeon removes the damaged ACL remnants and reconstructs a new ligament using a graft – commonly taken from the patient’s own hamstring tendon or patellar tendon (autograft)​. The procedure is done through tiny keyhole incisions using an arthroscope (a small camera) and specialized instruments, which means less tissue damage and smaller scars than traditional open surgery. The graft is securely fixed in place of the original ACL, and over time it heals into the bone, effectively becoming a new ligament​. ACL reconstruction is typically performed under regional or general anesthesia and usually requires an overnight hospital stay (some patients even go home the same day). After surgery, a structured rehabilitation program is crucial for success (see Recovery & Rehabilitation below). Modern arthroscopic techniques have a high success rate in restoring knee stability, allowing most patients – including athletes – to return to their previous level of activity after recovery.
